Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023 Toyota Corolla, Corolla Cross, Corolla Cross Hybrid, Highlander, Highlander Hybrid, Tacoma, Lexus RX350, 2023-2024 NX250, and NX350 vehicles that may have been incorrectly repaired under previous NHTSA recall number 23V-480. An electrical connection inside the steering column's spiral cable assembly may be insufficiently welded, causing the connection to separate and deactivate the driver's air bag. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Potential Consequences:
I was driving on I-290 toward Chicago when traffic began to slow. I also began slowing down when I was suddenly rear-ended by the vehicle behind me at a high speed. My car sustained significant rear-end damage, including a crushed bumper and a detached exhaust pipe that was dragging on the ground. The other vehicle involved was totaled. Police arrived on the scene and determined the other driver was at fault. While waiting for the tow truck, multiple tow personnel commented that, based on the severity and location of the impact, the airbags should have deployed — but they did not. I did not receive any warning lights, alerts, or messages before or after the impact. I did not hit my head, but it was a real possibility given the force of the crash, and I believe this created a serious safety risk. I purchased this vehicle in large part due to its reputation for safety, and in this situation, I felt unprotected. At this point, the vehicle has not yet been inspected by a dealer or manufacturer, but the police and insurance companies have been involved. I am concerned that the failure of the airbag system to deploy represents a defect or malfunction in the safety system. I am willing to make the vehicle available for inspection if needed.

Both dashboard screens (driver and center display) went black, and numerous warning lights appeared afterward, including oil pressure, engine, lane departure, tire pressure, and airbag (SRS) system malfunction. The vehicle accelerated involuntarily, the brakes did not respond, the steering became stiff, and the airbags failed to deploy upon impact with concrete and metal highway barriers. It does not sound loud enough to be heard under all conditions by other drivers or pedestrians. Similar vehicles come with two horns: one high frequency and one low frequency. How was your safety or the safety of others put at risk? The described condition certainly can put at risk the driver safety or others. Has the problem been reproduced or confirmed by a dealer or independent service center? The problem was reported to the Toyota Dealership. They said that they tested the horn sound, was compared to a known good model, and did not find a difference in the sound intensity or pitch. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, the dealer said that it tested the horn sound and compared it to a known good model. Were th. any warning lamps, mess. or other symptoms of the problem prior to the failure, and when did they first appear? Yes. When I first took delivery of the vehicle, the horn sounded louder.
