Safety Issue:

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Potential Risk:
An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.
FREE Recall Solution:
Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

Defect Description:

LATCHES/LOCKS/LINKAGES

Potential Consequences:

The contact owns a 2022 Toyota Sienna Hybrid. The contact stated while closing the sliding door, there was no safety mechanism to stop the door from closing on a person or object. The contact stated that approximately four months ago, her eldest son was entering the vehicle, he became stuck between the door and the door frame. The contact stated that on a later date while her youngest son was entering the vehicle, the close button was depressed on the outside of the vehicle, and without any mechanism to prevent premature closure, his arm became stuck between the door frame and the door while the contact was inside the residence retrieving additional items to take the children to school. The contact stated that her older daughter was able to remove him from the vehicle. The contact stated that her son sustained injuries to his arm and had marks all over his arm where it was stuck between the door and door frame. There was no medical attention provided but an ice pack was placed on the part of the arm that was stuck in the door. The contact was concerned about the safety risk posed by the failure and the lack of a sensor or mechanism to prevent the door from closing while a person was entering or leaving the vehicle. There were no warning lights illuminated during the failure. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 47,921.

Potential Consequences:

The contact owns a 2022 Toyota Sienna Hybrid equipped with Bridgestone Tires, Tire Line: Turanza L5100, Tire Size: T235/60/R18, DOT Number: [XXX] . The contact stated while driving 80 MPH, the rear passenger's side tire had a blowout. The contact stated she drove across the road and pulled to the side of the road. The contact stated that AAA assisted in replacing the tire with a spare tire. The vehicle was taken to Walmart the following morning, where the spare tire was replaced. Additionally, the contact stated while driving at an undisclosed speed, the TPMS warning light was illuminated. The vehicle was taken to Discount Tires, where it was determined that there was a hole in the rear driver's side tire. The tire was replaced. The contact stated that the front passenger's tire was also replaced at Walmart due to a pinhole. The dealer was notified of the failure. The tire manufacturer was not notified of the failure.
