FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Potential Consequences:
1. Other: Turn signal - when turning right with the signal activated, once you straighten the wheel, the turn signal stalk will reset to the middle but will activate the left turn signal. 2. Adaptive cruise control - For no reason the brakes will activate, there is no vehicle in front and the windshield is clean. 3. Blind spot warning - These BSW indicators will go off at random with no vehicles beside mine. This occurs more at night, but does happen during the day. When this happens at night the yellow indicator is so bright that you cannot even see what might (or is not) in your blind spot. This faulty indicator is going to get someone hurt as other drivers with the same issue might over correct.

Potential Consequences:
Speedometer and infotainment digital display. Speedometer changes from MPH to KPH, Infotainment display/Speedometer shuts down while driving. Not able to tell correct speed or see speed and critical information. Dealer said that there was no problem even after updating computer system. No other inspection has been done. No previous indications of problems, this first started 5-6 months after receiving the vehicle. This is a company vehicle that is tracked, daily I get alerts that the driver is doing speeds that are impossible due to traffic and I have had the problem replicated for me.
Corrective Action:
Speedometer and infotainment digital display. Speedometer changes from MPH to KPH, Infotainment display/Speedometer shuts down while driving. VISIBILITY/WIPER
Potential Consequences:
Wipers works 95% of the time. However, they randomly will not work some times and the time varys from 10 minutes to 12+ hours before they start to work again. This issue has been present since I bought the truck in July 23 and I've taken it to the dealer a half a dozen times for this issue but in all cases except once the wipers started to work before the tech could diagnose the problem. The one time they were able to work on it while it was not working they determined the lighting control module had reported failures. They replaced the module but I again experienced another incident where the wipers stopped working for 12 hours - this occured during the weekend when the dealer was not open. Dealer keeps stating they cannot replace parts unless they can prove something is broken so every time I drop it off even with video proof they say they cannot replicate the issue and send me on my way. A TAC case has been opened with GM and am awaiting feedback. The first time this happened I was driving 70+MPH on a highway when a sudden storm hit. I went to turn wipers on and they did not come on. I had to roll down the window and stick my head out to see to exit the highway.

This issue has been present since I bought the truck in July 23 and I've taken it to the dealer a half a dozen times for this issue but in all cases except once the wipers started to work before the tech could diagnose the problem. The one time they were able to work on it while it was not working they determined the lighting control module had reported failures. They replaced the module but I again experienced another incident where the wipers stopped working for 12 hours - this occured during the weekend when the dealer was not open. Dealer keeps stating they cannot replace parts unless they can prove something is broken so every time I drop it off even with video proof they say they cannot replicate the issue and send me on my way. A TAC case has been opened with GM and am awaiting feedback. The first time this happened I was driving 70+MPH on a highway when a sudden storm hit. I went to turn wipers on and they did not come on. I had to roll down the window and stick my head out to see to exit the highway.
Additional Notes:
Wipers works 95% of the time. However, they randomly will not work some times and the time varys from 10 minutes to 12+ hours before they start to work again. Potential Consequences:
Rotors have warped at 17,000 miles. Has been confirmed by dealership to be the issue. This is incredibly soon for this issue to arise, and driver has not driven erratically or applied brakes suddenly. Noticed when brakes suddenly started shaking entire vehicle when applied.
