POWER TRAIN

Potential Consequences:

This is the second time this happened in one week. Incident happened immediately after being sent home without repair by the dealership. Driving down the interstate, our car displayed the "Stop Safely. Vehicle will shut off soon." Red turtle light came on, and car lost all acceleration. We managed to pull over without being hit. Turned the car off and then back on. Check engine light was now on, but the car would drive. Hybrid battery was charged at 66%. In both shutdown instances, dealership says it is unable to replicate the problem, and therefore will not look further into the problem.

ENGINE

Potential Consequences:

We were driving on the interstate at approximately 70 mph when our car gave the warning, "Stop Soon. Vehicle will shut off." A turtle warning light also lit up. We pulled over to the side of the road and turned the car off. After a few minutes, we were able to turn the car back on and drive home, though the check engine light was then on. We took it to the dealer the next day, with the check engine light still on, and the dealer was unable to reproduce or diagnose the problem. They said there were so many warning codes that they could not determine what was the problem and that if they followed what the codes were telling them, they'd have to replace the entire wiring system of the car. They also said that one or more of the codes was related to the transmission. They drove the car extensively and couldn't reproduce the shutdown. They are currently still trying to diagnose the problem with the car at the dealership. There were no warning signs from the vehicle prior to the incident. The incident occurred right around the time when we would have depleted the all-electric charge and switched over to using gas.

ELECTRICAL SYSTEM

Potential Consequences:

Keys were inside car with only an infant inside and the doors were locked with no other key fob anywhere nearby. There must have been an issue with the car’s safety system to unlock the car in this situation. There was no 3 beeps that would unlock the car. Car was parked in Walmart parking lot in the heat. Remote start was on, key fob was in drivers seat. The last door to close was the drivers side rear sliding door. Wife called me from a shopper’s phone since hers was also locked in the car. I frantically drove with the other key fob a few miles to where they were to unlock. It was a traumatic experience for her and our son who was stuck inside the car. Thankfully I got there in time to unlock with the other key fob and everyone is okay.

UNKNOWN OR OTHER

Potential Consequences:

We have had our 2023 Chrysler Pacifica Plug-In Hybrid since [XXX]. This was a factory ordered vehicle. There have been no indications that there is any issue with the vehicle. About a week ago while driving the vehicle a warning came on the speedometer screen that said "Pull over safely the vehicle is about to turn off". The check engine light also came on. Once the vehicle was pulled over to the side of the road and stopped the vehicle turned itself off. After waiting a few minutes the vehicle was able to be restarted and driven about 5 minutes home. The check engine light remained on. We did not drive the vehicle again until we were taking it to the dealership to be inspected. The check engine light was on the entire drive from our home to the dealership, which is about 10-15 minutes. The vehicle has now been at our local Chrysler dealership for 6 days. The technician at the dealership is telling us the code stored in the vehicle memory means it needs a new transmission. We are being told they need to replicate the issue for the warranty to cover a new transmission being put in. If they cannot replicate the issue, we are to take the vehicle back and continue to drive it until it happens again and then immediately have it towed to the dealership so they can receive warranty approval to replace the transmission. My research has revealed this is a known issue in Pacifica Hybrids regarding stalling due to a transmission sensor. I called Chrysler to clarify the warranty process and if there is a recall on my vehicle. The warranty process is correct and there is currently not a recall for this issue on our Pacifica Hybrid. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Potential Consequences:

We have less than 3K miles currently on this vehicle. We were traveiling to the mountains for a vacation when the check engine light illuminated. Stayed on for 2 days and reset itself. We were in the Smokey Mountains and a warning came up to "Stop Safely Vehicle will shut off soon. Within a few minutes, all power was lost, all lights were inoperational including the 4 way hazards. After just a few minutes, the van restarted and operated as if nothing happened. The check engine light stayed on long enough for a Chrysler dealer to read the codes. Esentially, a problem with the High Voltage System Interlock Circuit A, Drive Motor A Inverter Performance are the major codes. A slew of others all related to this issue. The hybrid mechanic stated it is the same issues that plague the 2017-2021 Pacificas hybrids. Losing all power is not acceptable, especially in the mountains with a vehicle less than 4 months since delivery.

BACK OVER PREVENTION

Potential Consequences:

The contact owns a 2023 Chrysler Pacifica. The contact received notification of NHTSA Campaign Number: 24V436000 (Back Over Prevention) however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that the back-over prevention camera displayed still images, with blackouts, or was inoperable. The failure had become more persistent and then became a constant failure. The contact had to restart the vehicle to regain functionality or correct the malfunction. While turning off and restarting the vehicle, the radio was randomly activated without driver input. The contact believed that the failure had caused additional failures with other control panel functions such as the radio, heating, and the A/C system failing to activate as needed. There were no warning lights illuminated. The dealer was not contacted. The manufacturer was not made aware of the failure. The failure mileage was 14,000. VIN tool confirms parts not available.

UNKNOWN OR OTHER

Potential Consequences:

I was driving my vehicle in a local road at speed about 20MPH and suddenly got a warning saying "Stop safely, Vehicle will shut off soon". Then the gas pedal stopped working. It was on the middle of the road. I switched off my car and started it again a couple of times and that warning was gone but the engine light was on. I was able to drive the vehicle and so the next day I went to the dealer to find the issue. After 4 days the dealer said they couldn't replicate the issue and also the engine light was not on anymore and so they returned my vehicle without doing anything. After a week I was driving on a freeway with all my family members (6 of us), 75 MPH the same thing happened again. I immediately pulled over to the shoulder, since the gas pedal was not working. I switched off the car and switched on then I drove the vehicle. This happened at least 6 to 8 times on my 300-mile drive. It was really scary that the vehicle got a weird warning and the gas pedal stopped working in the middle of the road. At one place there was no shoulder, so I had to stop on the road itself. After coming back, I dropped my vehicle with the dealer to find the issue. It's been 2 weeks but they are saying they couldn't replicate the issue. I bought this as a brand-new vehicle on April 2023, and it's just 14 months old.

ENGINE

Potential Consequences:

On the afternoon of May 26, 2024, I received a warning "Stop Safely - Vehicle Will Shut Off Soon". I pulled to the side of the road but was still in an active traffic lane in a traffic circle and cars had to stop and/or swerve to avoid hitting my car. If this had been at night my safety would have been even more greatly compromised. The engine promptly shut off and would not restart. I was towed to the nearest dealer which still has not diagnosed or repaired the car. This may be related to recall 23V-010 and the ongoing RQ24004, of which the dealer had no knowledge. I took delivery of the car, a 2023 Pacifica plug in hybrid, on March 25th and it has less than 4000 miles on it.

Vehicle was purchased in February 2023, starting in January 2024 when vehicle was running and stopped, auto/start stop would engage but the entire vehicle would shut down and not restart. We would have to shift to park, turn off, remove foot from brake then wait 1-5 minutes to restart. Took the vehicle into Rochester hills Chrysler jeep dodge and after several days of troubleshooting they replaced the steering module and claimed it was faulty. 24 hours after picking up the vehicle the same issue happened again. Took the vehicle back in annd they showed 15 different error codes that came up. Left the vehicle there for service and after a week of troubleshooting they were finally able to recreate the issue. They claim it was the RF transmitter in the tailgate causing the issue. If you are experiencing the same issues I recommend asking about the RF transmitter and steering module.
