Defect Description:

VEHICLE SPEED CONTROL

Potential Consequences:

The driver was pulling into a friend's carport. After crossing a small curb her foot was taken off the gas and was moving to the brake. The car was going slightly uphill as slow as a car can go with no gas pedal depressed. The car then accelerated as if someone pressed the gas to the floor. In a matter of 8 feet it gained enough speed and momentum that when it hit our friend's parked car the 4 door SUV was pushed forward 4 feet. The gas pedal was never pushed by the driver. No failed component was ever identified by multiple inspections by the Toyota dealership, the insurance company, the body shop and the 3rd party company hired by Toyota to download the ACM data and the Vehicle Control History. We had filed a complaint with the Toyota Brand Engagement center hoping Toyota would make a proper investigation. A police report was made also. No one was injured but the homeowner was standing a few feet away. It could have been much worse. There were no warning messages, the emergency braking did not engage, and the airbags did not deploy. Earlier in the day the car's proximity sensors were beeping with no indication of what was setting them off. It took 3 cycles of park to drive to stop the beeps. The driver even walked around the car to try to identify what was setting it off. After the crash the passenger side rear window went down by itself and the rear liftgate went up. It took 7 weeks for Toyota Corp Legal to respond saying they found nothing wrong. To this day they have not shared the Vehicle Control History raw data with us. Their response made no mention of them looking into a failure of the ADAS system with regard to the Cruise Control or any other sensors that send signals to the throttle. Moonroof spontaneously exploded while driving on the highway. The moonroof was closed, no one was trying to open it. All windows were closed, no one was trying to open them. My husband was driving and differently we heard a loud explosion which sounded like a shotgun going off and thought someone was shooting at us. I looked behind the car and could see glass flying everywhere. We safely pulled over and the entire moon roof had randomly and spontaneous exploded. No collision. TIRES

Potential Consequences:

My girlfriend's father was driving at normal driving speed on the highway when the tire blew out for no reason. There are multiple blown out sections of the tire on both the internal and external sides of the tire that would appear to be a structural failure. This was definitely not a puncture due to the failures completely around the entire circumference on both sides of the tire. The tire was inflated as per spec. This caused vehicle instability and was a dangerous situation so he had to quickly pull to the side of the highway. The tire is available for inspection. I have not yet taken the car or tire to the shop for inspection. There were no warning lamps or other symptoms prior to the blowout. The tire size is: 235 / 55 R 19 101V

I was stopped at a red light at an intersection, when a car pulled up next to me, and the driver rolled down the window to tell me my vehicle was on fire. I got out of my vehicle while the driver of the other car called 911. Brookline Police & Fire came, firefighters extinguished the fire, and one of the emergency services responders had a wrecker come & take the car away. The car was towed to Watertown, then collected by Safety Insurance, which got a repair estimate of $95,000. The car was deemed a total loss and is in the possession of the insurance company. When the vehicle caught on fire, it had 3,627.4 miles on it. I did not see any warning lamps, messages or other symptoms of the problem prior to the fire. My safety and the safety of others was at risk due to the sudden and unexplained fire.

Potential Consequences:

The contact owns a 2024 Toyota Rav4 Prime. The contact stated while driving at approximately 45 MPH, there was an abnormal explosion coming from the roof of the vehicle. The contact then became aware that the panoramic sunroof had shattered without impact. An unknown local dealer was notified of the failure and referred the contact to the Insurance Company for assistance. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 500.

Potential Consequences:

Main infotainment display doesn't switch to night/dark screen even though the settings is set to automatic mode. As a result the display is extremely bright especially in dark and rural areas blinding the driver. It caused me to hit a curb and blow a tire. I went to a Toyota dealer 3 times but they didn't not fix it. This occured on August 13 2024 at around 9:30 PM. I filled a complaint with TOYOTA but every time I called they promise they will address the issue but it's now 2 months and they have not done anything. I am not sure what to do next but the display remains an issue .

Potential Consequences:

Panoramic moon roof exploded while driving with no impact to the glass. The explosion sounded like a gun shot inside the car. This sudden explosive noise put the driver, and all other vehicles on the road, in danger due to the panicked reflex of the driver. Upon inspection, it's evident that the glass shattered along all 4 edges rather than from a single point of impact. This would indicate an issue with the glass, the installation, or both. Vehicle was taken to dealer and customer was told it would not be covered under warranty.

I have oiwned the car for 3 months and there has not been a problem previously. As I was pulling into a parking space at work, going less than 5 mph, i took my foot off the accelerator and coasting in while intending to brake, stop, and turn the car off. Suddenly, the car accelerated forward quickly, propelling the car over a tall curb, over a small grassy area, and onto the open parking lot. I subsequently applied the break, the car stopped, I drove back to the same spot. and was able to park and turn the car off. A report was filed with Toyota, completing an unintended acceleration form (UA), and Toyota filed TAS case number 24212013.

Bought brand new car from Toyota. While driving on Highway at 60 mph car suddenly went dead and catastrophic warning showed up on dash. Was barely able to pull over to side of road. Very dangerous. Had car towed to Toyota who informed me that battery was defective in cold weather and went dead. Battery replaced. Recall inspection is warranted.
