Safety Issue:

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."

Potential Risk:
Driving with a deactivated stability control system increases the risk of a crash.
FREE Recall Solution:
Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

Safety Issue:

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Sienna vehicles equipped with seating for eight passengers. An incorrect seat belt webbing guide may have been installed on the outboard second-row seat belt assemblies.

Potential Risk:
During seat belt pre-tensioner deployment, the incorrect webbing guide may damage the seat belt webbing, causing it to tear and increasing the risk of injury during a crash.
FREE Recall Solution:
Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.

SERVICE BRAKES

Potential Consequences:

I am writing to formally raise a serious and unresolved safety issue with my 2022 Toyota Sienna. The braking system on the vehicle behaves abnormally: when the brake pedal is pressed, the van does not stop immediately. In fact, it momentarily accelerates before decelerating, an extremely dangerous malfunction that has nearly led to multiple accidents. I have brought this issue to your attention several times: • In 2022, I contacted O'Regan's Toyota, the Halifax dealership, which has been unable to diagnose or repair the issue to date after several visits. • In 2023, I contacted Toyota Canada directly. I was referred to the dealership in Halifax. • The Halifax dealership has been unable to diagnose or repair the issue to date. • Despite the unresolved nature of this problem, the dealership has recently asked to charge me diagnostic fees for the same issue, one that remains unsolved and is clearly a manufacturer-level safety defect. This is not a minor inconvenience; it is a fundamental failure of the braking system and presents a critical safety hazard. The fact that this issue persists without a resolution, and that I am being asked to pay diagnostic fees for a problem that Toyota has not addressed, is unacceptable.

SERVICE BRAKES

Potential Consequences:

When braking, the vehicle "releases" and lurches forward at a faster rate, but then catches again and continues to brake as it was before. It does not flash or light up on the dash with any issues when this happens. We have tried to find how to specifically recreate this issue to show to the dealership, but it just happens randomly anywhere from 2-12 times a day. This is especially terrifying coming down steep hills or taking quick offramp with stopped traffic in front of you. It began about a year and a half ago and used to take place once every few weeks, it is now happening daily, with increased "release" strength and time. It is still only about 1-2 seconds of released brake pressure, but again is terrifying when behind the wheel and expecting your vehicle to continue to brake as normal while foot is on the pedal. The brake pedal does not depress further or move from its position when braking. The car is acting and performing just as it should be 'normally', except that the brakes give way for 1-2 seconds.

Potential Consequences:

Unexpected sudden acceleration (when braking) as I was slowing down from exiting a highway, just as I was approaching full stop, it felt like the regenerative braking stopped grabbing momentarily and I violently lurched forward for what seemed like a feet or two. I thought it was a fluke back then and thought nothing of it. It just happened again today as I approached a stop sign - again as I neared complete stop with brake depressed it felt like the brake stopped working for a second and the car again lurched forward violently as if I had lost control for what seemed like a foot. Would have hit the car in front of me if there was one.

Potential Consequences:

seat retractor failure- white plastic part off and visible , seat is making popping noise when seat is being adjusted. Noticed this when i picked up from dealer after oil change. I feel this is a driving safety concern. Larry H Miller American Toyota (Albuquerque, NM) service manager states it's safe to drive, however I feel this is a huge safety concern. The seat was in working order did not have this issue during drop off of service. However I believe this is a failure on Toyota engineering. I would like some guidance on how to get this repaired as they want me to pay $2,000.

Potential Consequences:

On May 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a T-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station. My wife was unable to brake and stop the forward progression of the vehicle. My wife steered to avoid hitting trees but could not avoid the brick of the fire station wall. The airbags did NOT deploy. By the summer of 2024, the Toyota dealership was finally able to replicate it, and they fixed it with a “computer update” as best I understood. The problem was fixed for a while but began again a few months later. The frequency at this time , early 2025, is every 1-2 days. I have returned multiple times to Toyota, and they have no fix since they have not been able to replicate it this time. I have pointed out to them that they did replicate it when they had previously fixed it. This issue presents a danger when the driver is coming to a stop. The first time it happened my reaction was to move my foot to the accelerator and I jumped a small curb. Although I am prepared now for this to happen, it is possible my instinctive reaction could happen again, resulting in a collision or injury. There are multiple forums and I see multiple complaints on this website, but I see no investigation cases. Please help me understand why this safety issue has not been investigated.

The issue was related to partial or improper function of the DSM which caused a drain on the battery and battery cell damage requiring full replacement. The fix was a software update to prevent recurrence which cost several hundred dollars (according to the Toyota Service Bulletin or TSB-0089) along with approx $200 for diagnostic analysis. The TSB work would have been covered if we had not exceeded 36,000 miles. We had the vehicle for 3 years and 1 month. I do not understand the mileage impact to the coverage of a firmware update as this was related to a no start condition and could have occurred when we stopped at a rest stop area as we were traveling across country at the time. I am very disappointed with the failure and the impact of getting several jump starts for our vehicle as well as the couple days waiting to get scheduled into a dealership and 4 hours to get fixed.
