Safety Issue:

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."

Potential Risk:
Driving with a deactivated stability control system increases the risk of a crash.
FREE Recall Solution:
Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

POWER TRAIN

Potential Consequences:

While steadily accelerating on a freeway onramp, the vehicle suddenly accelerated, surged and gained speed rapidly without my control. I took action by braking and slowing the vehicle to a safe freeway speed. Thankfully no one in my vehicle was hurt and no other vehicle was near during the incident. There were no warning messages. The first time I experienced this unintended acceleration was the first month when this brand new vehicle was purchased from a local Toyota dealer. I was turning into my driveway from a residential street , the car suddenly accelerated while I was in the middle of turn. I immediately/emergently stopped by stepping on the brake, I could have crashed into the power pole if I did not stop. Because this was a new car, I convinced myself I was unfamiliar with its controls. But since the second occurrence, I am greatly concerned about this random "unintended acceleration". Windshield cracked center of windshield, at the bottom, from a small pebble hitting it about 2 inches from the bottom. Car has approx. 7,500 miles and is barely 7 months old. Have seen this problem with dozens of owners in the same location of the windshield. Windshield also is leaking (see photo) from under the rearview mirror plastic cover.

Corrective Action:

VISIBILITY/WIPER

Potential Consequences:

The vehicle was parked in a parking lot for several hours. After returning to the vehicle, it was found that a crack had appeared on the drivers side of the windshield that was approximately 12 inches long, starting at the bottom left of the windshield, and moving upwards. The crack is right through the middle of where the driver must look when driving down the road, which is a safety hazard. The salesman at the dealer stated that he hadn't heard about this problem, but in looking online, many others have complained about the same vehicle having windshield cracking issues. The vehicle is scheduled to be inspected by the dealer tomorrow.

Potential Consequences:

Very fragile windshield. Although this may be hard to measure, I only had a very small rock hit and the windshield cracked. I've been driving for 25 years (16 cars and suvs) and never had such a small impact create a significant crack. It created a visibility issue and I'm worried about future issues.

Twice in the (almost a)year I’ve had the Venza I have gone to accelerate from a stop sign and the car started to go then the gas pedal went all the way to floor and nothing happened - then when I tried again the car accelerated as expected. The first time (@ approx 4000 miles) I was turning left with oncoming traffic (of semi truck plus others) coming and had it happened a few seconds later - I would have been in a serious accident. Second time was just over 10000 miles on 1/12/2021

SERVICE BRAKES

Potential Consequences:

Pulling into a parking spot (under 5 mpg), brakes did not engage. Car drove up onto the sidewalk. Had to put car into reverse to avoid hitting the building. While in reverse, brakes would not engage as well had to turn off the vehicle to stop it after it hit the curb 20ft behind the vehicle. Had vehicle towed to the dealership. They could see where the brakes were engaged 3x, but could not replicate the issue. Although there was snow, ground was not icy or slippery.

EXTERIOR LIGHTING

Potential Consequences:

Headlights: They have a shadow blocking full view. They are not shining equally on both sides. Instead the beam drops off the field of vision abruptly and you now faced with a blind spot of darkness. Basically, if something or someone came out in front of you, you would not see them until it was too late. For instance, driving in the neighbor alley without lighting, if a child, animal or ball came out in front of you, you would have no shadow or warning that you were about to come into contact with them. With other vehicles the light sprays off into the distance so you can see a wider field of vision. I find this extremely dangerous, unsafe for me and others! If I use my high beams the problem is solved but that is not a proper solution. I have spoken to Toyota dealer x2 and their answer is... That's the new LEDs. I have called Toyota Corp and they have registered my call. I truly believe this is a matter of urgency for the safety of human life.

Potential Consequences:

Windshield cracked along drivers side with no known rock impact and no evidence of a rock hit. It appears the windshield just cracked. The crack was about 2 feet long. Car was only a few weeks old when this happened.

EXTERIOR LIGHTING

Potential Consequences:

Low beam LED headlights have shadows and dark spots projecting on to the road. Very distracting and looks very similar to animals or people as they come into view in the dark. Have had to swerve several times only to realize it’s the shadows in the headlight units. I have read many other owners complaining of identical issue on Toyotanation Venza forum. Really need a fix for this. It is not an issue that appears over time, this is happening straight from the factory with 0 miles on the car.

Potential Consequences:

Don't recall anything unusual hitting my windshield (other than sandy grit), but when I drove it today there is a small chip and 12" crack. I have driven cars for 35 years and have never had such a fragile windshield on a car. I think it is defective, because I read about a bunch of other people having the same issue on this Toyota Nation site (and also Facebook)- https://www.toyotanation.com/threads/windshield-repair.1695878/ It is not very safe to have people drive around with windshields like this, especially if it is the fault of poor manufacturing. And even though you are supposed to get it fixed with insurance, I am not in a place right now to afford the deductible to get it fixed.
