Safety Issue:

General Motors, LLC (GM) is recalling certain 2020 Chevrolet Bolt EV and 2021 GMC Canyon and Sierra 1500, 2500 and 3500, Yukon, Yukon XL, Cadillac Escalade and Escalade ESV and Chevrolet Blazer, Camaro, Colorado, Equinox, Silverado 1500, 2500 and 3500, Suburban and Tahoe vehicles. Incorrect bolts may have been used at one or more fastening locations to secure the seat belts to the vehicle. If an incorrect bolt was used, the seat belt may not be properly attached to the vehicle.

Potential Risk:
If a seat belt assembly is not properly attached to the vehicle, the seat belt may not properly restrain an occupant in the event of a crash, increasing the risk of injury.
FREE Recall Solution:
GM will notify owners, and dealers will replace the suspect bolts, free of charge. The recall began January 29, 2021. Owners may contact GMC customer service at 1-800-462-8785, Cadillac customer service at 1-800-458-8006, or Chevrolet customer service at 1-800-222-1020. GM's number for this recall is N202322230.

Defect Description:

ENGINE

Potential Consequences:

On [XXX], I was driving my Blazer up to our local Museum to grab some information. The Museum is about a mile and a half from my house. When I had left my house, the car was driving like normal. I pulled into a parking spot at the Museum, and I went to shut the vehicle off, and I thought maybe the check engine light had come on but wasn't completely paying attention to the dash. About 10 minutes later I went to restart the vehicle to leave and I noticed the check engine light on. Since the vehicle seemed to be driving okay I thought I could make it home and check the car's computer to see what the issue was. As I was leaving the parking lot I stopped at the end of the parking lot to look before pulling out onto the road and the Blazer said shift to neutral and restart the vehicle. When I did that the vehicle would not start. It would just crank and crank and then it would finally start for a few seconds. I was going to back my vehicle up out of the road but each time I would try the car would die. I am very thankful this did not happen on the highway or even on the freeway. A friend and myself had to push my vehicle back into a parking spot. The DTC code is a P023F. When this happens, you can't technically shift the vehicle into neutral to push it out of the way because the car has to be running to shift. I was able to force it into neutral, however. I made an appointment with Awesome Chevrolet in Chehalis WA and had to have it towed to the dealer. They stated it was the fuel pump module. I had at that time 18,388 miles on the odometer. Unfortunately, I no longer feel safe with this vehicle in case it happens again. Where I am hours from home or on a 2-lane road or freeway. The fuel pump module failure appears to be a known issue with GM vehicles. The fuel pump module was replaced and was able to be replaced under warranty. I hope that it does not happen again. P0128, AC not working, Steering Assist Reduced, and Temp Gauge not working. Problem caused by faulty ECT wiring. First the problem is the ECT is a two wire (signal + and -) 26 gauge wire running through the ignition coil wire harness. Ignition coil signals are very high voltage and fairly fast frequency which causes a lot of EMI which is easily picked up on a 2 wire voltage based wire set. The ECT wiring should have been a twisted shielded pair if running next to Coil wiring or they could have used a single wire resistance thermistor, but they should have just routed the signal a different way to begin with. This is a design issue and it should be a recall...maybe a class action lawsuit is needed to force them to fix it correctly. The FSB (field service bulletin) fix blames loose connector terminals and it patches the computer software to not include ECT wire failures in the Steering, AC, and temp gauge inhibit logic (overtemp readings will still fail off these functions). Defect Description:

ELECTRICAL SYSTEM

Potential Consequences:

Upon driving my vehicle twice it shut off on the roadway once on an incline when a message of 2 keys detected and flashed on the dashboard prior to the vehicle shutting off. Vehicle would not restart until 15 minutes later it restarted but the dashboard kept flashing 2 keys detected. There was only one key in the vehicle at the time and the key fob has had the battery replaced upon the first incident which was on 11/14/2024 and the second incident was on 11/16/2024.

Corrective Action:

Defect Description:

SERVICE BRAKES

Potential Consequences:

The brakes failed on my SUV as I was driving on a city street. Yes it is available for inspection. Losing my breaks almost caused a collision with the car in front of me but due to the distance between the cars and the low speed at the time, I was able to avoid collision and detour into a parking lot where I cruised to a stop with the help of a parking lot space concrete stop. The problem was not reproduced by me but the service center computer diagnostics confirmed break issues. The service center says that the cause was low voltage of the car battery which affected the brakes. This is a very dangerous situation if the vehicle can lose complete braking ability due to a low voltage battery. This is the reason for reporting the issue. They replace the battery and said that should fix the issue. They said the only recourse to avoid this happening again is to replace the battery around every 2 years. The issue was inspected by a service center owned by the manufacturer of my SUV. The warning lamps only occurred as the brake system failed. A message of "Brake System Failure" appeared and the "Check Engine" light came on. Within the previous week of the issue the SUV also had other electrical issues such as the Climate Control interface went out and the rear camera also went out.

The Radiator has a crack in the upper left front of the core causing the fluid to leak and lose pressure causing the engine to overheat. This is a manufacture defect that has not been addressed as of 10/16/2024. The leak was discovered by a certified GM mechanic at a dealership when doing routine maintenance and inspection. The vehicle has never had any indication or advisory light that would alert the operator. As the engine overheats it can cause many of the other components of the vehicle to disable, including but not limited to the ADAS systems as well as power steering and/or collision safety components. This is a manufactures defect with the aluminum radiator that needs to be addressed immediately. Given the information from multiple GM dealerships the amount of newer low mileage Blazers that have been confronted with this problem is unprecedented. This not only creates a danger for the passengers of the vehicle but also every other vehicle on the road.

Power Liftgate randomly opens when driving or parked. Even when the power switch is turned to off the liftgate will unlock randomly either parked or driving. I have contacted the dealer and GM no recall currently exists. Dealer recommended replacing the latch mechanism which I have ordered to replace myself.

Potential Consequences:

Instrument cluster completely blacked out, providing no information on vehicle speed or status of operation. Operation of a vehicle without instrumentation would seem to be a safety risk. Chevy dealer diagnosed problem as IPC error U0155, installed updated software to correct the problem. The repair was not covered by the extended warranty I purchased through General Motors when I bought the car. I think a defect such as this should be the manufacturers responsibility, I believe GM has a service bulletin out for this problem covering several models of their vehicles. This should be upgraded to a recall.

I have a 2021 Chevy Blazer which has had to have two radiators replaced with less than 50,000 miles on the vehicle. The first time was in March of 2023. I took my vehicle in for an oil change and the technician told me the radiator was cracked and all of the coolant was drained from the vehicle. It must have just run out because I regularly check my fluids and the vehicle was not overheating prior to the appointment. The vehicle had about 32k miles on it, so the radiator was covered under warranty. Now in April of 2024 with 49k miles, I take my vehicle in for another oil change and the radiator is cracked, again, with coolant leaking. The technician told me two things that are concerning about this. 1.) This is a known issue on the Blazers that they’ve done multiple repairs on. 2.) There is a piece of plastic under the radiator that catches the coolant when it leaks, making it hard to notice an issue because the coolant doesn’t pool on the ground underneath. My service advisor went to bat for me against GM, who partially paid for the second radiator. This brought my installation cost to $235, even though it was no longer under warranty. Radiators cracking is a serious safety issue because if the driver doesn’t notice the engine overheating, it could cause a vehicle fire putting the occupants and surrounding environment in danger. There are no warning lights on the vehicle to tell you if there is low coolant, other than noticing the temperature gauge when it is too late. I don’t know if GM didn’t install rubber bushings to absorb the impact of bumps on the road, or if it needs some rubber washers on the bolts, but something is missing. Online forums will confirm that this is an ongoing issue for owners of the Chevy Blazer

I was driving on the highway when my car started operating like it was in neutral. RPMs was high but the car started to stall and slow down and because of it I had to immediately pull over. The car would not go into gear. I attempted to put it in drive ajd press the acceleration pedal but it just revved. Same with putting it in reverse. It's a safety concern because my car could not maintain highway speed and abruptly went stagnant. I had the vehicle towed to dealer and they replaced the entire transmission system. The car always had a shudder feel to it when accelerating from 0 onwards to 40 mph.The day this occurred there was no blatant warning signs.

The failing component is the ESC/power steering. It is available for inspection. It is currently at Bayway Chevrolet in Pearland, Texas. My safety and the safety of others is at risk when the power steering fails not allowing me to turn out of the way from road hazards or even make legal turns. This could result in an auto accident causing bodily injury or death to me or other drivers. Yes, a service shop in Alvin was able to see/reproduce the errors. No, however the vehicle is currently in the shop being looked at by service techs. There were no warning lights, messages, or other indications prior to [XXX] around 2pm EST, when this happened. The vehicle was driven home from work that evening. The vehicle was not driven all day [XXX]. Today, [XXX], I drove the vehicle to the service shop in Alvin, Texas to replace the battery. Around 10:45am EST, the errors/messages came on. I drove back to the battery shop in Alvin and they ran the codes. This is where they were able to feel the steering tighten up. I drove the vehicle to Bayway Chevrolet for repairs. Once I receive an estimate or repair invoice, I would be happy to send that over.
