Defect Description:
AIR BAGS
Potential Consequences:
The safety system needs serviced/repaired and the alert has been present for months, and investigated by GM licensed dealership but not repaired, after several requests. The safety of drivers and passengers has been neglected and the high risk for injury and death while occupying the 2021 Chevy Tahoe. Also on 06/11/2025 the EPS Power Steering system failed, with the system alert showing up on the driver's dash. This is a cause for major concern and is noted that 2021 Chevy Tahoe vehicles have an outstanding recall for those affected. The licensed GM dealership was contacted on 06/12/2025 and again, denied the repair and/or ability for the vehicle to be operated safely. The Tahoe has been taken to the shop over the last 12-16 months and as of this day, occupants of this vehicle are at very high risk for injury and death due to the service safety system alert and the failed EPS system. I am compelled to take additional action, to prevent injury and death unless these issues are able to be immediately resolved.

Defect Description:
ENGINE
Potential Consequences:
While starting from a stop the engine stuttered and resulted in a major loss of power. Luckily this occurred pulling out of our driveway on to a quiet neighborhood street. Several warning lights and messages went off including the engine and transmission system warning, “service ESC”, “Service Traction Control”, and the emission system warning. After driving it to the dealership at 30mph since the shaking and banging noise was so bad, the dealer diagnosed it as a cylinder 6 caused by a failed lifter and lifter rod. Resulting repair involved lifters and rods in that same bank at a $3500 repair. The vehicle was just over 4 years old with 79,250 miles.Repair invoice available upon request.

Defect Description:
ELECTRICAL SYSTEM
Potential Consequences:
Instrument panel has failed twice and is unresponsive. The gauges move and stay to the left after moving left to right. All lights begin to cycle through error messages. I have video of the issue both times it happened. When it occurs, you don't know your speed you have to guess, the full tank reads empty etc., and it is dangerous to back up. The vehicle has been diagnosed by one independent mechanic and is currently at the dealership. There was no warning before the instrument cluster failed other than a cycling through of warning messages service transmission check 4X4, service suspension system, service Power Steering Drive with care. -The center screen goes in and out. -When in reverse, the rear camera is black, the screen blinks intermittently. - Dealer has stated that the wires are put on incorrectly- if that is the case, I am not sure who would have done that other than the manufacturer. -previously reported there was a strong gasoline smell in the cabin that made it really hard to breathe. Dealer corrected. Right passenger side is intermittently wet with no cause from the interior creating stale smells, making it hard to breathe. It is important to note this is the side where the electrical wires are "put in incorrectly" according to the dealer. I also have a video of what occurs

Corrective Action:
Instrument panel has failed twice and is unresponsive. The gauges move and stay to the left after moving left to right. All lights begin to cycle through error messages. EXTERIOR LIGHTING
Potential Consequences:
We were driving our 2021 Chevrolet Tahoe (purchased Dec 2020; odometer currently right under 40,000 miles) out of town this past weekend and I noticed the headlights were not illuminating the road ahead very well once the sun went down. They seemed like they were pointed down and when i turned on the high beams, those were also lower than how the low beams are usually pointed. We were stuck in the middle of nowhere and I persevered with low lighting onto our destination. After researching the issue on the internet days later, I found a Tahoe Owners' forum/thread [XXX] ) where several other 2021 Tahoe owners had experienced the exact same issue as ours had. The culprit appears to be a sub-component of the headlight adjuster mechanism, but GM/Chevrolet does not offer a replacement for either this small white plastic piece that is failing and the only repair option they have for this issue is complete replacement of the entire headlamp assembly ($1,350 MSRP). This is a dangerous problem that occurred randomly on a vehicle with low mileage and appears to potentially be a common failure for other owners of the same headlamp design. I have not taken this vehicle into the dealership for inspection, as I expect they will only replace the entire headlamp assembly for $$$$. ENGINE AND ENGINE COOLING
Potential Consequences:
The contact owns a 2021 Chevrolet Tahoe. The contact stated that while her husband was driving 40 MPH, the "Service Stabili-trak Soon" message was displayed on the instrument panel. The contact stated that there was an abnormal knocking sound coming from the engine and the contact's husband pulled over and restarted the vehicle. The contact stated that after the vehicle was restarted, the failure persisted. The contact's husband pulled into a gas station and called an independent mechanic. The mechanic advised the contact's husband to safely drive the vehicle home and to then take the vehicle to his shop the next day for a diagnostic test. The vehicle was driven to the independent mechanic and was diagnosed, and the contact was informed that cylinders #1 and #7 were not functioning as needed. The contact was then informed that the engine lifters needed to be repaired. The manufacturer was notified of the failure and parts were ordered to repair the vehicle. The vehicle remained in the possession of the mechanic and was not yet repaired. The failure mileage was approximately 75,400.

Defect Description:
FUEL/PROPULSION SYSTEM
Potential Consequences:
Check engine light comes on. Car suddenly stalls and engine shutters, shuts off and on. Goes into limp mode as engine shutters at about 15 miles per hour. GM has not included this 2021 Tahoe which is the same exact condition under Product Safety Recall N212332040 "Stall While Driving". That recall was issued at the same time or month as this Tahoe was built and sold and should have been included. This Tahoe was towed to the dealer and GM has not been able to supply parts. The Tahoe will soon have been at the dealer for 60 days with no solution. The recall build dates should be expanded to include all GM vehicles with this defect. It is explained as: "The fuel pump power control modules in these vehicles may contain a supplier manufacturing defect that can cause the fuel pump to fail or operate intermittently, interrupting the flow of fuel to the engine. If the engine does not receive adequate fuel, the vehicle may unexpectedly stall. The driver may see a check engine light, receive an OnStar notification that a Diagnostic Trouble Code has been set, or experience a rough running engine. The vehicle may also be placed into a reduced power mode. A vehicle stall without warning could increase the risk of a crash, especially at high speeds"

FORWARD COLLISION AVOIDANCE: WARNINGS
Potential Consequences:
On April 25th, 2025 we were taking our kids to their after school activities when the SUV started to shake, and accelerate very fast by barely pushing the gas pedal, the ETS light along with the forward collision warning also appeared on the dashboard along with the check engine light. This prompted my husband to get our vehicle towed to the nearest shop close to the street where our vehicle started having problems. After several days of diagnostics the shop let us know our vehicle needed the following repairs: Bank One lifters replacement, head gasket service, and a water pump replacement, we ok'd the repairs. Although these components were replaced, we were informed on May 15, 2025, that the vehicle continues to misfire and now requires a camshaft replacement as well. The total cost of the repair has now risen from $5,182.16 to $13,549.79, which is a significant financial burden as the vehicle is just outside of warranty coverage. We are aware that there is an active recall covering similar issues for 2021–2024 Chevrolet Tahoes equipped with 6.2L engines. However, our 5.3L engine model is experiencing the same mechanical failures, including misfires, knocking noises, low RPM idling, and performance degradation. There is growing evidence that the valve lifter issues are not limited to the 6.2L engine variant. Our experience—and the experiences of others reported online—suggests that the 5.3L L84 engine may also suffer from the same design or manufacturing flaws. We respectfully urge NHTSA to investigate whether this issue warrants expanding the scope of the existing recall to include the 5.3L engine models, and we ask that GMC/General Motors be encouraged to provide assistance. Given the severity and known pattern of these issues, we believe GMC should cover at least 75% of the repair costs, or ideally, offer to perform the repair at no charge. Thank you for your time and consideration.
