ELECTRICAL SYSTEM
Potential Consequences:
The radio emits an uncontrollable extreme static. The radio volume, power, or any control will not stop the noise. The sound is so loud the car is not drivable. We removed the fuse to the amplifier and this fixed the uncontrolled static but the radio does not work. This is a huge safety issue as the driver is unable to hear any sirens, horns, or cars outside the vehicle. This issue needs to be addressed in a recall. This is not normal wear.
Corrective Action:

Potential Consequences:
The contact owns a 2016 Honda Accord. The contact stated that the vehicle failed to start with the check engine warning light and battery warning light illuminated. The vehicle was towed to the local dealer who diagnosed that the starter was faulty and needed to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure, but no assistance was offered. The contact indicated that a service bulletin was issued by the manufacturer that addressed the failure. The failure mileage was 75,000.

Potential Consequences:
Rear seat passenger seat belt stopped working. Passenger indicated seat belt wouldn't buckle. Dealer inspected the seat belt and ordered the seat belt buckle on 2/3/2023. Part was ordered at highest priority, but estimated release date from Honda's vendor is 5/28. Four months without critical safety part seems excessive. Honda Customer Service is unable to do anything to expedite availability of part from their vendor.

Potential Consequences:
Audio main head unit (Honda p/n 39171-T2F-A71) produces loud static from radio speakers. The noise becomes extremely loud. The loud static continues even if the audio system power is turned off. The noise is so loud it is disorienting. The only way to stop the noise is to pull off the road and turn off the engine. The loudness of the static puts the safety of the driver at risk. The problem is intermittent and cannot be reproduced on demand. I provided the Honda dealer with an audio/video tape of the problem. This tape was used by the service department to diagnose the problem. I paid $170 for the diagnostic test. Vehicle was inspected by the Honda dealer. Honda dealer advises the part is on backorder. Tentative availability is April 10, 2023. Vehicle was inspected by two independent auto stereo businesses. They were unable to repair because OEM wire harness is needed. There were no warning lamps or messages prior to audio main malfunction. The problem first appeared in November 2022. The car is not driveable when the static occurs. Since I cannot drive the car with the loud static, I paid the Honda dealer $85 to disconnect the audio main head unit. There is no static, but there is no radio, backup camera, bluetooth functions, etc. The problem was reported to American Honda Motor Customer Care on February 17, 2023. Assigned Case No. 13663941. Requested ETA of backorderd audio main head unit. Advised that this problem was a SAFETY issue. I received a call from American Honda Motor Customer Care on February 20,2023. They have documented the problem and closed the case.

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
Potential Consequences:
Inoperable backup camera causing Rear Cross Traffic Warning and Parking Collision warning malfunction - due to a camera plastic mounting tab breakage issue. This puts the safety of other cars or people behind the vehicle at risk when backing up. There were no warnings of the failure - my wife discovered it when reversing one day and had a near miss.I looked at it and the backup camera is no longer in the proper position. The car only has 18K miles on it. I took it to the Honda dealer and their service team tells me one of the camera mounts is broken and it happens all the time. They tell me the Accord backup camera usage is extremely high due to mounting tabs breaking. I spoke with my local Honda garage and they told me they see this issue all the time. I called multiple Honda Dealer Parts Dept's across the country and they all tell me usage is very high and the cameras are on back order. There were four Technical Service Bulletins issued on this 2016 Accord to check for backup camera usage: MC-10187541-0001, MC-10189132-0001, MC-10189839-0001 & MC-10190945-0001 - in February through March 2021. The 2017+ Accords had been recalled by NHSTA for "inoperable" camera software as it was considered a safety issue in the past. Most recently (in 2023) Honda FIT and HR-V models were recalled by the NHSTA related to backup camera issues. The design of the backup camera was also changed as the part number for the replacement does not match the original & yet Honda is not retrofitting the new parts for owners. The Honda part numbers for the camera are as follows: Original in the car: 39530-T2A-A310-M2; First replacement for the original P/N 39530-T2A-A31; Current replacement (February 2023) P/N 39530-T2A-A410-M1 (Sony VCB-H3D8). I had my original backup camera replaced by my Honda dealer - the original part was kept by the dealer to be sent back to Honda of America (per the dealer). I did keep pictures of the old camera as well as the replacement.
Corrective Action:
Inoperable backup camera causing Rear Cross Traffic Warning and Parking Collision warning malfunction - due to a camera plastic mounting tab breakage issue. This puts the safety of other cars or people behind the vehicle at risk when backing up. There were no warnings of the failure - my wife discovered it when reversing one day and had a near miss.I looked at it and the backup camera is no longer in the proper position. The car only has 18K miles on it. I took it to the Honda dealer and their service team tells me one of the camera mounts is broken and it happens all the time. They tell me the Accord backup camera usage is extremely high due to mounting tabs breaking. I spoke with my local Honda garage and they told me they see this issue all the time.
