FORWARD COLLISION AVOIDANCE: WARNINGS
Potential Consequences:
On July 2, 2025, while driving at approximately 8:35 PM my 2024 Toyota Corolla Cross suddenly and unexpectedly came to a complete stop. The vehicle's Pre-Collision System (PCS) activated without any obstacle, pedestrian, or vehicle in front of me. I was decelerating at about 15–20 mph in clear weather and road conditions. The abrupt stop was extremely dangerous and could have caused a serious rear-end collision had another car been behind me. This created a significant safety risk to me and to others. I had already completed the Toyota recall related to the pre-collision/braking system weeks prior. Despite this, the malfunction still occurred. The issue has not yet been confirmed or reproduced by a dealer, but I have contacted Toyota and scheduled a full inspection. The vehicle is available for inspection upon request. There were no warning lamps, messages, or error alerts before the incident. The car simply braked suddenly without warning. Out of fear for my safety, I have since disabled the PCS system. I am now afraid to drive the car, especially at higher speeds, and have requested that Toyota escalate the matter for further resolution or potential buyback.

January of 2025 the vehicle was put in drive and the vehicle shut down. "Power steering low egine stopped" message appeared. I Put the vehicle in park and started the car back up. I took it to the dealership. The battery was low they charged it and released it back. The vehicle was brought back shortly after that due to the vehicle being put in park and shutting down displaying the same "low power steering. Egine stopped" message. Along with "EPB stopped Incompletely". Brought it back in and the battery was low and they replaced it. A few months later the vehicle was brought in due to multiple braking events either when pulling forward with no acceleration (nothing was around the vehicle) or putting the vehicle into reverse with no acceleration (nothing behind the vehicle). During these events the car made a loud grinding sound and displayed BRAKE across the dash. The 3rd time at the dealership was due to the car shutting down when put into park and braking when reversing out of a parking spot. The code "epb stopped Incompletely" and the BRAKE message appeared.The dealership checked for codes but the car didn't have any saved. They can't recreate it. They had a feild tech come out but I'm unsure of what was found because they haven't released it. As of June 22, 2025 I was driving and the car braked on me while going 40 mph. The seat belt locked up and threw me and my passenger forward. A red BRAKE message appeared and the car sounded an alarm. The vehicle was brought back into the dealership on June 23, 2025 the dealership turned me away. Corporate started a case.

On [XXX], at the intersection of [XXX] and [XXX] in Los Angeles, [XXX] was operating the subject vehicle when it was involved in a hit-and-run that resulted in a secondary collision with multiple vehicles. Photos (attached) document the significant impact. Emergency responders were required to extricate [XXX]from the vehicle, during which they informed her that the airbags failed to deploy—a fact also documented in the official LAPD report. Brakes were malfunctioning and did not work. I was traveling South of Hwy 99 in the middle lane and noticed the vehicles in front of me were slowing down and braking. I attempted to applied the brakes but it was not working. I swerved to the left to avoid a collision with the vehicles in front of me. As I swerved to the left onto the fast lane, I hit the vehicle in that lane. Myself, along with the 3 passengers were transported to the hospital. The vehicle is towed and has not been inspected by the insurance company. Police was called to the scene. There are no recalls that I know of on brakes for my vehicle.

The headlights on this vehicle are not safe. They have a shadow , or curtain over the top half of beam. This severely curtails your ability to see safely down the road. Going up to a hill, your ability to see road hazards are almost nill. You cannot see hazards to the side of vehicle , or above, either. My wife is a school bus driver, and is aware you should be able to see 15 seconds in front of you, or a city block. This vehicle totally fails to illuminate anything near that. And you cannot read street signs, unless they have their own lighting. We took it to the dealer. They adjusted the lights, which didn’t solve the problem, and now we are getting flashed by other drivers, and are causing road rage. Please help!
