Potential Consequences:

I was driving down the road, slowed down and moved over to the left side of the lane as I was passing a bicyclist. The car interpreted the bicyclist as a car and applied AEB. The AEB was not fully applied, thankfully. The car did not come to a complete stop. If it did, I may have been rear ended by the person behind me.

ENGINE

Potential Consequences:

Car has broke down a total of 4 times since I have gotten it from Toyota back in December of 2023. I have a dash-cam now that has footage of the car breaking down. Toyota fails to acknowledge my concerns and comes up with excuses and tells me to wait until it happens more frequently. I would love to have this car inspected, but as Toyota has stated they cannot get the issue to repeat. My safety for others and myself feel uneasy with the whole ordeal as it happens at the most random times. The first few times my car has broke down, the steering wheel locked up and forced itself all the way to the left on its own causing me to hit a curb and median. Manual control over the steering wheel was not possible. Luckily with this occurring both times I was going a slow speed. The other two times I had full control of the wheel but my engine would stop and the engine stopped message would appear on my dash screen. Vehicle has been inspected by the dealership 4 times with no luck on repeating the issue. Breaking down presented a dash message of engine stopped, abs system stopped, power steering stopped and would quickly cycle through warnings and shutdown.

STEERING

Potential Consequences:

Vehicle Information: * Make: Toyota * Model: Corolla Hybrid AWD * Model Year: 2024 * Current Mileage: [14,000] * Mileage When Problem Noticed: ~8,000-9,000 miles Incident Description: My 2024 Toyota Corolla Hybrid AWD has a severe steering defect. After a right turn, the steering wheel consistently fails to return to center, remaining 5-15 degrees (up to 20 for sharp turns) off-center to the right. It does not self-correct. Occasionally, this also occurs after left turns, but less frequently. The vehicle requires constant, active manual correction to stay straight. If I release the wheel, the car can drift right, posing a significant collision risk. The steering feels "floaty" and lacks the expected resistance to return to center. This issue began abruptly around 8,000-9,000 miles. Multiple dealership visits and alignment checks have not resolved it. No prior accidents or modifications. The Lane Tracing Assist (LTA) system functions smoothly, keeping the car centered. However, immediately after disengaging LTA, the manual steering defect reappears, indicating an underlying mechanical problem, not an electronic one. Safety Concern: This defect creates a serious safety hazard. It demands constant driver attention and effort, leading to distraction and fatigue. This compromises directional stability and significantly increases accident risk, as the car will veer off course if not continuously corrected. I urge NHTSA to investigate this defect. Similar "sticky steering" or "poor return to center" issues are reported in other modern vehicles, and Toyota itself issued CSP 22TE02 for a related "slower to return to center" issue in 2018 Camry models, proving this is a known type of defect, not normal operation.

SERVICE BRAKES

Potential Consequences:

Dear Better Business Bureau, I am filing a formal complaint against Toyota Motor North America regarding a serious and unresolved safety defect in my 2024 Toyota Corolla LE (VIN [XXX]). The vehicle experienced two incidents of sudden unintended acceleration—the second, on [XXX], resulted in a collision. Despite my repeated efforts to resolve the issue, Toyota has failed to respond adequately or provide critical diagnostic data. In both incidents ([XXX] and [XXX]), while stopped at a red light, the vehicle suddenly surged forward despite my foot being on the brake. On [XXX] this caused a collision. My vehicle has remained undrivable since. Toyota conducted a diagnostic inspection on May 29 under Case #[XXX]. I’ve repeatedly requested access to the raw data from this inspection to understand the defect, but Toyota has not responded. Emails to the assigned case manager, Ms. Cynthia Ware, have gone unanswered. When I was directed to reply to a support email, it bounced back as a “do-not-reply” address, redirecting me to a login page which falsely claimed I had no active cases. This created an endless communication loop. Toyota’s lack of transparency and support is unacceptable—especially given the severity of the issue and the fact that the vehicle is still under warranty. I am requesting: Immediate release of the raw diagnostic data from the May 29 inspection A prompt resolution, including full repair, replacement, or buyback Clear, responsive communication from an authorized representative This is not only a financial burden, but a serious safety risk. I respectfully request BBB’s assistance in securing a resolution and holding Toyota accountable. Thank you for your time.

Additional Notes:

SERVICE BRAKES

Potential Consequences:

I was leaving out of the parking lot of the shops Santa Anita. I was parked underneath by [XXX] and proceeded to to leave and turn out right towards the wide open parking lot. At the time I was leaving, it was completely empty. This parking lot is wide open as I was leaving and I’m going no more than 15 mph when suddenly a beeping sound goes off and the car slams on its brakes on its own! I didn’t even have enough time to stop on my own and look to see what the alert sound was. The car stop and jerked me so hard and slammed forward so hard. Thankfully I was in an empty open parking lot. That would have cause a very bad car accident and I would have been badly hurt and the car that would have slammed into me from behind! This has left me with so much anxiety and I am now very scared to drive this car at all even. I haven’t even had this car for a year and this is the second time I’ve had an issue with it. And I tried explaining to the dealership that for the past couple weeks to month that when I drive the car, it has this pulling feeling when I’m coasting. It has never felt that way before. This is supposed to be a new car. It doesn’t even have 15,000 miles on it yet! INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

SERVICE BRAKES

Potential Consequences:

I was able to distinguish today the car noises my vehicle is making happens when I break. Like a “eeeeek” sound. My mileage is under 8,000, had the car almost a year. Also I notice my wheel stiffens. I’m able to verify it happens when I merge - specifically when the lane dashes end. The wheel somehow gets tight even when driving with both hands. As if the vehicle gets confused. This has happened about 5 times this year but every time it happens- I brace myself for the vehicle to crash since I can’t control the wheel, no matter how strong I try to gain control. The wheel gets too tight to steer. These episodes lasts about 3-4 seconds. I am going to take it to the dealership on April 1, 2025. Also there were no messages on my dashboard when these episodes occurred.

ENGINE

Potential Consequences:

Upon startup in cold weather conditions, a brief but loud grinding/rattling sound will occur. The noise is related to an inadequate spring inside of the intake camshaft gear which will not push a small cylinder in place to lock the intake camshaft gear in place. When not in its proper place, the gear will rattle in its housing, causing the brief noise. If the gear rattles against its housing too hard overtime, it can break and cause catastrophic engine failure by sending metal pieces into the engine, including pistons and rods.
