Safety Issue:

Chrysler (FCA US, LLC) is recalling certain 2022-2025 Pacifica and Voyager vehicles. The right and left side curtain air bags may have insufficient pressure retention from improperly sealed seams. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."

Potential Risk:
An air bag with insufficient pressure retention increases the risk of ejection and injury during a crash.
FREE Recall Solution:
Dealers will replace the side curtain air bags, as necessary free of charge. Interim notification letters, notifying owners of the safety risk, were mailed beginning July 1, 2025. A second notice will be sent once the final remedy is available. Owners may contact Chrysler customer service at 800-853-1403. Chrysler's numbers for this recall are 06C, 54C, 55C.

Safety Issue:

Chrysler (FCA US, LLC) is recalling certain 2020-2023 Pacifica and 2020-2021, 2023 Voyager vehicles. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."

Potential Risk:
An air bag that does not deploy during a crash increases the risk of injury.
FREE Recall Solution:
Dealers will secure the connector, free of charge. All vehicles have been remedied. Owner notification letters were mailed October 22, 2024. Owners may contact FCA customer service at 1-800-853-1403. FCA's number for this recall is A8B.

Safety Issue:

Chrysler (FCA US, LLC) is recalling certain 2021-2022 Dodge Durango, 2021-2023 Chrysler Pacifica, Jeep Grand Cherokee L, 2022 Ram 1500, 2500, 3500, Chrysler Voyager, 2022-2023 Jeep Compass, Grand Cherokee, Wagoneer, Grand Wagoneer, andRam Promaster vehicles. The radio software may prevent the rearview image from displaying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Potential Risk:
A rearview camera image that does not display decreases the driver's visibility, increasing the risk of a crash.
FREE Recall Solution:
Dealers will update the radio software or the software will be OTA, "Over-The-Air," free of charge. Owner notification letters were mailed beginning July 9, 2024. Owners may contact FCA US, LLC customer service at 1-800-853-1403. FCA US, LLC's numbers for this recall are 66B and 79B.

Safety Issue:

Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.

Potential Risk:
An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.
FREE Recall Solution:
Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. FCA began notifying owners on February 23, 2023. Owners may contact FCA US, LLC customer service at 1-800-853-1403. FCA US, LLC's number for this recall is 03A.

Chrysler (FCA US, LLC) is recalling certain 2022 Voyager and Pacifica vehicles. The windshield wiper arm nuts may be improperly tightened, which can cause the wipers to not function properly.

Potential Risk:
Improperly functioning wipers can reduce the driver's visibility in certain weather conditions, increasing the risk of a crash.
FREE Recall Solution:
Dealers will tighten the wiper arm nuts, free of charge. Owner notification letters were mailed August 30, 2022. Owners may contact FCA US, LLC customer service at 1-800-853-1403. FCA US's number for this recall is Z80.

Safety Issue:

Chrysler (FCA US, LLC) is recalling certain 2021-2022 Pacifica and 2021 Voyager vehicles. The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."

Potential Risk:
An unsecured seat increases the risk of injury in a crash.
FREE Recall Solution:
Dealers will inspect and, if necessary, replace the second-row seat cushion frame, free of charge. Owner notification letters were mailed April 7, 2022. Owners may contact FCA US, LLC customer service at 1-800-853-1403. FCA US LLC's number for this recall is Z22.

ENGINE

Potential Consequences:

Loss of power while driving at high speed, almost caused crash. The dealership technician inspected and updated PIM software and claimed that it won't happen again. However, the van shut down again while driving on a highway on the 2nd day after the "PIM software update". The car is currently with dealership with no information shared regarding what is causing engine shut down.

ELECTRICAL SYSTEM

Potential Consequences:

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at 35 MPH, the vehicle stalled. The contact steered to the side of the road and received assistance from a Police Officer who disconnected and reconnected the battery. On another occasion, while driving at 75 MPH the contact stated that the vehicle stalled. The transmission warning light was illuminated. The vehicle was steered to the side of the road where the battery was disconnected and reconnected again. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was 17,602.

Corrective Action:

7/19/23 - Dash warning to pull over because car was going to turn off (from recall software update). (Image attached) Unclear how long I had and upcoming road was more dangerous, so I pulled over in an area with highway speed traffic and only half a shoulder. Car did not reset on first attempt to restart and stuck in "turtle mode" (<10 mph). When I called the dealership, they originally offered me the roadside assistance number, then talked me through reset over phone when I pushed back that I was not in a safe spot with cars flying past me. I was able to drive <10 minutes to dealer after car reset. They were able to get codes and confirm car is undrivable and I need a new transmission. As of 7/28 they still have my car with no update on when part will be shipped. Prior warning: On 7/11, dash showed "put in P to park" alternating with "press brake and push button to start" while driving down the road (note D for drive in lower right on attached images, also photo of car still on/in run). This started at the same time the car bucked and the check engine light came on solid. (Another image shows the red battery warning light in upper left that came on when car was put in park as other messages continued to alternate.) I pulled over and turned off car. While waiting and thinking about what to do next, the car restarted on its own. (It is possible I pressed button 2x to turn off as I recall it didn't react. But restart was a full minute later!) Restarted. Dealership seemed unconcerned, despite the known issue with Pacifica hybrids. Same issue occurred 7/12. On 7/13 the radio rebooted while driving down the road with no warning. (Radio issues have been ongoing and may be related to overall electrical issues?). On morning of 7/19 when final issue occurred, the radio didn't work at first. On 7/14, I pushed dealership to get software recall update that day as it did not feel safe to drive with the ongoing issues. Recall software update was in place before 7/19 incident.

(Another image shows the red battery warning light in upper left that came on when car was put in park as other messages continued to alternate.) I pulled over and turned off car. While waiting and thinking about what to do next, the car restarted on its own. (It is possible I pressed button 2x to turn off as I recall it didn't react. But restart was a full minute later!) Restarted. Dealership seemed unconcerned, despite the known issue with Pacifica hybrids. Same issue occurred 7/12. On 7/13 the radio rebooted while driving down the road with no warning. (Radio issues have been ongoing and may be related to overall electrical issues?). On morning of 7/19 when final issue occurred, the radio didn't work at first. On 7/14, I pushed dealership to get software recall update that day as it did not feel safe to drive with the ongoing issues. Recall software update was in place before 7/19 incident.

Additional Notes:

7/19/23 - Dash warning to pull over because car was going to turn off (from recall software update). (Image attached) Unclear how long I had and upcoming road was more dangerous, so I pulled over in an area with highway speed traffic and only half a shoulder. Car did not reset on first attempt to restart and stuck in "turtle mode" (<10 mph). When I called the dealership, they originally offered me the roadside assistance number, then talked me through reset over phone when I pushed back that I was not in a safe spot with cars flying past me. I was able to drive <10 minutes to dealer after car reset. They were able to get codes and confirm car is undrivable and I need a new transmission. As of 7/28 they still have my car with no update on when part will be shipped. Prior warning: On 7/11, dash showed "put in P to park" alternating with "press brake and push button to start" while driving down the road (note D for drive in lower right on attached images, also photo of car still on/in run). This started at the same time the car bucked and the check engine light came on solid. My car had less than 500 miles on it. While driving down the interstate, it suddenly stopped accelerating and a warning to “pull to a safe place the vehicle will soon shut off” appeared. The vehicle shut off. It was towed to the dealership. After almost a week with the van, the dealer said nothing was wrong and they sent me back on my way. 5 miles down the road and on a busy road, same thing happened. Van stalled and was again towed to the dealership. This time they said it needed a new PIM system. It was at this time that I became aware of NHTSA Campaign Number: 23V-010. I was sold this car on June 14 after Chrysler was aware of the issue.

This required an immediate emergency stop and crossing multiple interstate traffic lanes without any forward propulsion power. We were able to restart the vehicle. Upon restart the system malfunction indicator lamp was illuminated. Vehicle was driven at slow speed to the nearest dealer service center for evaluation and service.

I was driving this vehicle, a 2022 Chrysler Pacifica Hybrid this evening, July 7th. The battery was operating on a charge of 1% and the gas tank was 1/3 full. I charged the battery earlier this morning but only for several minutes. This is a rental vehicle I have used for the last week with no problems. Vehicle was idle for about 3 hours prior to this incident. After starting the car at about 8:30 pm, I drove no more than several miles and received a warning that I needed to pull over “immediately” and that a vehicle shutdown was “imminent.” I did so and turned off the vehicle. I turned it on again and the “Check engine” light was on but the notice I received earlier was gone. I was able to drive the rest of the way home which was about 10 miles. I searched the vehicle and VIN later and determined this vehicle was under NHTSA Recall # 23V-010 which had been completed. I am reporting this defect because it seems to be similar to the recall shown as completed on the vehicle in January 2023.
