Chrysler (FCA US, LLC) is recalling certain 2022-2025 Pacifica and Voyager vehicles. The right and left side curtain air bags may have insufficient pressure retention from improperly sealed seams. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."

VEHICLE SPEED CONTROL
Potential Consequences:
Vehicle suddenly stops and shows the message "Stop Safely Vehicle will shut off soon". Then the vehicle will lose all power and vehicle can crash with little prior warning when on the highway. This is very unsafe and a recall was performed on 1/17/2023 but the problem still persists over and over and is very unsafe. Once the warning happens about 2-5 seconds later the vehicle will shut-off and can crash when driving. This incident has happened several times and is not isolated to once.
Corrective Action:

Vehicle Will Shut Off Soon" Within minutes and while she was in the middle lane of the Freeway and the speed shutdown to 19mph causing cars behind to almost collide with our van. Drivers showed road rage towards her. She was able to coast to the freeway exit. We got the van towed to a dealer where it is now. I have read many reports of this same thing happening to may others. Something needs to be done about this.

On three separate occasions (December 2023-February 2024), while driving in temperatures below freezing, our 2022 Chrysler Pacifica Hybrid dashboard gave the following warning, along side the illumination of a red turtle warning light: "Stop Safely. Vehicle will shut off soon." The vehicle then lost the ability to accelerate, and the speed dropped significantly and immediately. Two of the three times, my one and three year old were in the vehicle and there was no shoulder on the 50mph road we were traveling on after sunset. Fortunately, the three times this happened, we were less than a half mile from our home and we coasted downhill to the bottom of our driveway. Our vehicle has now been at a certified Chrysler dealership for three full business days, and the technician "has no idea what is wrong," "is still working on it," "has no idea how long it will take to fix," and "has no rental or loaner cars available" for our family while we wait for our family's mode of Transportation to be returned. They claim they are unable to recreate the issue, so they cannot fix it. We showed them a picture of the dashboard, but that is not enough for them. It is currently 72 degrees in Dayton, OH, and the issue has never occurred in temperatures above 30 degrees. Thus, I doubt they will be able to "recreate" the issue until next winter. They've offered no other options or resolution until they can "recreate the issue."

Ever since delivery of our vehicle it would seem to have a hard shift between the electric and gas motor. Recently, we were driving one morning and noticed that when the gas engine kicked in, the RPM’s were much higher than usual. Later that day we received the “Stop Safely the Vehicle Will Shut Off Soon” message along with a check engine light. Shortly thereafter the vehicle went into limp mode. We are very lucky this happened in town as opposed to on the highway or in a remote area where this would pose an even more serious safety risk. Especially with young children on board. We then brought the vehicle to the nearby dealer where they found P0A43-00 “Drive Motor "A" Position Sensor” and C0063-9A “ Yaw Rate Sensor-Component or System Operating Conditions” codes present. After nearly a week of troubleshooting and opening up a “STAR” case, the dealership is now in the process of acquiring and replacing the transmission on our vehicle. This seems to be related to the recent recall 03A “Loss of Motive Power” and Chrysler TSB 21-008-23 affecting the internal transmission wiring connector. Our recall was completed promptly after issuance but this did not address the underlying issue of the internal transmission wiring connector. Our van is just over one year old and has under 21,000 miles on it. Needless to say this is unacceptable for a vehicle of this age, use, and especially MSRP! We purchased this vehicle as a safe option for our family and now we have lost confidence.

Loss of drive power while at high speed on the highway. Warning popped up saying car will lose power soon, with a turtle icon. Had to pull over 3 lanes to shoulder. Could have been dangerous, Chrysler known issue - they had recall 03A to make sure the warning turns up, but didn't fix it.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Potential Consequences:
While travelling on a controlled-access highway at 70 mph with the Adaptive Cruise Control active, and no other vehicles or objects nearby, the braking system engaged spontaneously (without driver input) and forcefully, for approximately .5 seconds, also engaging the anti-lock brakes. The rapid deceleration was forceful enough to snap my family of 5 all into their seatbelts. The speed of the vehicle slowed approximately 10 mph or more during this incident. My family and I were fortunate enough that no vehicles were behind us, no collision occurred, and I was able to maintain control of the vehicle though I was very much surprised. No warning indicators nor messages appeared. I took the vehicle to a servicing dealer three days after the incident for inspection/diagnosis. 2nd and 3rd row seats of the van do not allow for proper installation of children’s car seats due to the non adjustable and non removable headrest. 3rd row headrest folds forward making the issue even worse. Tried multiple car seat manufacturers and per the manufacturer’s installation guide the pacifica seats will not allow for correct and safe installation. Installation instructions show the car seat back must be flush with the vehicle seat. No known problems with other vehicle manufacturers when researching this issue

Potential Consequences:
While driving on the expressway at expressway speeds we got a message on the information system stating the engine will shutdown soon, pullover safely as soon as possible (paraphrased). The van started to lose power almost immediately upon receiving the message. We were lucky to pull over on a multi lane expressway since we were already in the right lane. It appears the gas engine did not engage when the battery was almost out of power (1%). On the side of the road, it took two times to turn the van on and off before the gas engine started. We were able to make it home. The next business day we contacted the dealership and had it towed there for service. After evaluation I was told the entire transmission needed to be replaced since a generator for the electric motor is housed there and is apparently the cause of the problem. At this time my vehicle is still with dealership awaiting parts. I did research online and see the message we got was the focus of a recall (to display the message) but the cause of the mechanical\electrical failure was still unknown from Chrysler/Stellantis. This is unacceptable. We could have easily been in a crash due to the issue. If they can't fix the actual problem, the message needs to display much sooner while the battery still has sufficient power such as 10%. I believe we had 1% indicated on the driver display at the time of the message.

09/17/23 Was driving 65mph, car was slowing Down in front of me and turned cruise control off. Immediately car started stalling. Was in middle lane with cars on both sides. Turned on flashers and no power in gas pedal. At about 30mph, was struck from behind by Jeep! Third car behind struck Jeep and Jeep struck me again. There were injuries and a death. My car was towed. 10/02/23; Now car is in body shop. Contacted Chrysler, they inspected car, but did not drive it. 10/12/23 Just received email stating that they found no problem. They stated that they are not responsible as they found no evidence of manufacturing defect or recall having contributed to this accident. Insurance companies and police are still investigating.
