Safety Issue:

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."

Potential Risk:
Driving with a deactivated stability control system increases the risk of a crash.
FREE Recall Solution:
Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.

Safety Issue:

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Sienna vehicles equipped with seating for eight passengers. An incorrect seat belt webbing guide may have been installed on the outboard second-row seat belt assemblies.

Potential Risk:
During seat belt pre-tensioner deployment, the incorrect webbing guide may damage the seat belt webbing, causing it to tear and increasing the risk of injury during a crash.
FREE Recall Solution:
Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.

SEAT BELTS

Potential Consequences:

After starting the van, an error pops up: "SRS Airbag System Malfunction Visit Your Dealer." This occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

Corrective Action:

Our 2022 Toyota Sienna would not start after arriving at a doctor’s appointment. Contacted active ToyotaCare for towing - no one showed up. Called a friend to jump start van; consulted the owners’ manual and performed jump. Van started but could only be driven about 100 yds in parking lot. Van stopped; multiple error codes showed up when trying to jump start again. ToyotaCare didn’t call again or show up so we ended up paying for tow service to the dealership - Haley Toyota of Richmond. Dealership checked vehicle; concluded the battery had an internal weld problem but that the jump start was not performed properly even though the manual was followed. There are no jump start instructions in the engine compartment. We were given a Tech Bulletin instruction sheet with more accurate information. We were advised the Engine Control Unit was burned out, causing all the error codes and would not be covered under warranty because “we” did not perform the jump start correctly. The cost of repair is almost $1100.00 but the battery, which started all the issues, is covered under warranty. The Tech Instruction sheet indicates to me that Toyota was aware of the inadequate jump start instructions and failed to inform customers of the possible issues. 3rd row seats when folded in well: They don't stay put in well and they come out with a bang upon applying a sudden break or going over a speed hump. Why is it a problem: 1) if a pet is there on the back, it could cause a serious problem 2) It makes a big bang when came out of the well and It make the driver think like they are rear ended and stopping their vehicles suddenly. 3) any light weighted articles fly to the front creates a possible hazard. What am I expecting: they stay put in the well and locked. What did I do? I spoke to the dealer and they acknowledged the problem and simply said that's how it is designed with no solutions.

Potential Consequences:

The contact owns a 2022 Toyota Sienna Hybrid. The contact had ordered the vehicle with modifications due to being a disabled veteran from a specialty vehicle modification for disabled persons manufacturer. The contact had picked up the van from a dealer and was driving approximately 60 MPH when the smell of something burning filled the passenger cabin. The contact pulled over and called the dealer who took the vehicle back and determined that the fuel line was rubbing against the rear passenger side tire. The vehicle was repaired. The contact stated that also the "check battery" light had illuminated, the light blacks out the entire instrument cluster and cannot be reset. The contact stated that he took the vehicle back to the dealer and they could not repair the warning light failure. The dealer had diagnosed that the main batteries were getting overheated making the vehicle dangerous to drive. The manufacturer had not been informed of the failure. Potential Consequences:

The contact owns an Convaid Car Seat, Model Number: 3365, Model Type: Carrot 3, Manufactured Date: May 15, 2018. The car seat was being utilized in a 2022 Toyota Sienna. The contact stated that the release button on the crotch restraint buckle, where the two arm and chest belts buckled, were cracked into two pieces rendering the car seat unusable. The failure occurred inside of the vehicle. The child was not in the car seat at the time of the failure. The manufacturer was made aware of the failure. The car seat was not replaced.

Potential Consequences:

When applying the brakes the vehicle doesn’t seem to want to stop and almost feels as if it’s trying to accelerate before finally catching and stopping. Typically it’s a pretty fast occurrence but now it’s taking longer to stop. It is available for inspection. It caused me to run a stop sign today and go into a persons yard before the van finally came to a stop. No service dept has looked at it yet. The problem doesn’t seem to be able to be reproduced on command and only happens at random. The vehicle has had no inspection and has no warning lights on during the time I’ve had the vehicle.

The brakes randomly give out for a second even while applying constant pressure on the brake pedal. It feels like the car lurches forward when the brake loss occurs. Even though it only lasts a second, it is terrifying and there have been a couple incidents when I was coming to a stop at a red light and I almost hit the car in front of me. This has also occurred in my 2021 Highlander Hybrid. It luckily has never happened during high speeds, but it’s scary enough going 30 MPH.

car seat gets loose easily and quickly with simple vibration down the road. example going over speed bumps and appropriate speed causes the seat to oscillate making seat belt strap loose that keeps the car seat in place. in one instance the strap was so loose i could not use the rotate function without the possibility of the car seat falling out of place. this is the second unit i have had with the issue manufacture is not moving to fix issue.

Our 2022 Hybrid Toyota Sienna will randomly have a loss of brakes for several seconds while in the middle of a stopping situation. The vehicle seems to lurch forward with the brake pedal going soft. After cramming my foot as hard as possible into the brake pedal I have been able to get it to stop so far, however my stop distance is much longer than anticipated. It seems to be getting worse in the past week with 2 of these episodes occurring. In the past year it has happened approximately 5 times. Each time is highly distressing. We have taken it to one Toyota dealership so far that could not find anything wrong with the brakes and have another appt in July with a second Toyota dealership. I find similar experiences described online from other 2022 Hybrid Toyota Sienna owners. I feel an obligation to report this because it is life threatening to be without brakes for even a few seconds while driving. There are no warning messages that appear when this brake failure occurs and it has not been reproducible.

While slowing to a stop with steady pressure on the brake pedal, the braking action will momentarily give out, resulting in the vehicle lurching forward beyond the anticipated stopping/slowing distance before the braking action re-engages. This has resulted in near collisions with cars, bikes, and pedestrians as the vehicle suddenly stops slowing without warning and unexpectedly increases stopping distance. The problem is intermittent and difficult to reproduce, but has occurred repeatedly over at least 6 months of ownership. Going over any kind of bump, crack, pothole, etc seems to increase the likelihood of occurrence, but it also happens on dry, flat pavement.
