On March 4, 2020, Toyota expanded the recall to include certain 2014-2015 Toyota 4Runner and Land Cruiser, 2018 Avalon, Corolla, 2014 FJ Cruiser, 2017 Sienna and Lexus 2018 ES 350, 2018-2019 GS 300, 2013-2014 GS350, 2014-2015 GX 460, IS 350 and LX 570, 2014 IS F, 2018-2019 IS 350, LC 500H and LS 500H, 2013-2015 LS 460, 2015 NX 200T and RC350, 2017 RC 200T and RX 350. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2018-2019 Toyota Tundra and Sequoia vehicles and 2019 Toyota Avalon and Avalon Hybrid vehicles. The air bag electronic control unit (ECU) may erroneously detect a fault during the vehicle start-up self check. If this occurs, the ECU may not deploy the airbags as intended, in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 208, "Occupant Crash Protection," and 214, "Side Impact Protection."

Potential Consequences:
We were driving down an unpaved road at 8 mph in 4WDh in our 2019 Toyota Tundra when I heard a couple popping sounds from inside the car. Then it stopped. I opened the passenger side door to get out and check on the truck, and flames were pouring out under the engine. There were 3 of us in the car. We immediately got out and tried to pour water and throw sand on the fire. It grew very quickly, so we ran away. The car was immediately engulfed in flames like something out of a movie with black smoke going high into the air. Within 5-10 minutes, the truck was totally gone. There were no warning signs/lights other than the 2 pops and then fire/explosion. We are very lucky we did not die. We bought it brand new. It was the first new car we’ve ever owned. It was impeccably cared for. We always brought it to the Toyota dealership in Eugene, OR for service. It was there about a month prior to the explosion. During that visit, they replaced the battery as well as dealt with the halogen light recall. Other things were more routine maintenance. We are in shock and do not want this to happen to others. We also do not know what our next steps are, but we wanted your agency to be aware of this issue. The local dealership said it must have been a car defect or bad workmanship at our last service. Either way, it was terrifying.

Sunroof shattered while driving on highway. Noise similar to a gun being fired, caused momentary disorientation; however, was able to keep vehicle on the road. No warning, no other vehicles around that could have kicked up a rock, debris, etc. Smooth conditions on the road (no potholes, bumps, etc.). Sunroof was in the tilt up position, side windows down, and rear window down - even temperature outside and inside (74 degrees F). Small glass "shards" in eyes and hair; however, most of it went up/out. Vehicle gets inspected tomorrow (09-21) for insurance purposes.

Potential Consequences:
A technical safety bulletin was issue for this issue (T-SB-0036-19). On a recent trip in the Great Smokey Mountains National Park, my wife and I were pulling a travel trailer with LED lights and noticed that the trailer disconnected warning come on. We thought it was a loose cable and pulled over to check the plug. We started home after our trip and had this occur several times on step grades. The scary issue is the lights continue to work indicating that it’s still connected, however when I checked the manual brake controller there were no brakes! There were no areas to pull over. Once we were able to pullover safely, we checked the connection again and it was still secure. This occurred again on I-40 as we approached and accident scene with sudden braking. Then it disappeared again. We made it safely home and I started researching the issue and found the bulletin. I took it to Toyota to complete but it’s still an optional bulletin. This is an enormous safety issue considering a 5400 pound truck capable of towing a 9200 pound trailer at highway speeds with no braking or access to the first step to stop trailer sway.

Several trailers were used to verify. I have attached the fix but Toyota says its warranty issue. This is a "safety" issue. I had my 3 year old son in the truck when i had to apply the brakes and couldn't stop in enough time. The truck brakes could not handle the weight of the trailer hence why the trailer brakes were needed. Please prevent a death.

Potential Consequences:
The contact's husband owns a 2019 Toyota Tundra. The contact stated that after her husband had refueled the vehicle, the vehicle failed to start as needed without warning. The contact pushed the vehicle into the gas station's repair garage where the mechanic informed him that the fuel pump was defective. The contact's husband then called the dealer where they informed him of NHTSA Campaign Number: 20V682000 (Fuel System, Gasoline) which the dealer related to the failure; however, the VIN was not included in the recall. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 36,348.

The contact's husband owns a 2019 Toyota Tundra. The contact stated that after her husband had refueled the vehicle, the vehicle failed to start as needed without warning. The contact pushed the vehicle into the gas station's repair garage where the mechanic informed him that the fuel pump was defective. The contact's husband then called the dealer where they informed him of NHTSA Campaign Number: 20V682000 (Fuel System, Gasoline) which the dealer related to the failure; however, the VIN was not included in the recall. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 36,348.

My son was driving to Mineral Wells to get something to eat on Wednesday 11/17/21 at 8:00 at night on FM 52 South. All of a sudden the steering wheel falls in his lap. He picked it up and shoved it onto the column and was able to steer the truck onto the side of the road without hitting anything. He looked underneath the steering wheel assembly and 2 of the 4 ubolts where missing. Where did they go - if they were ever there in the first place. The steering wheel has had some vibration and we have had the truck front end aligned and the wheels balanced. I had it in not long ago for an inspection/oil change/tire rotation. After I left the dealership the wheel felt a little loose and there was some vibration but I thought it was the road and it did stop vibrating. I also drive a Prius and I thought maybe I'm just not used to the way the truck feels. I told my son when I got home that it felt loose and he said he would keep an eye on it. Then Wednesday night the steering wheel falls off and the whole column falls out. What is this? Another issue is when the Valdosta Toyota (GA) dealer attempted to fix the fuel pump recall on 22 JUL 2020 it was assembled incorrectly. I called the dealership and told them to come and get my Tundra because gas was dripping onto my driveway. They sent a tow truck and it was supposedly fixed and they brought it back to me in about 2 hrs. It really does run the way it did before they “re-fixed” it. Very often now when it is driving I feel something like a failing fuel pump. It fades away but it is not comfortable taking it long distances. It feels like there is a clog in the fuel system or something, although there is not. I have 33,400 miles on it today and I can’t help but wonder if Valdosta Toyota didn’t just exchange the original fuel pump with a different used one?
