Defect Description:
SERVICE BRAKES
Potential Consequences:
Some portion of the brakes master cylinder degraded into small pieces and were found in the brake lines by a Ford Dealership technician. While traveling on highway at highway speeds (70mph), about an hour into a trip, the brakes locked on which caused vehicle to travel into the median and slide sideways. The vehicle had to be coaxed (put in manual mode), to get it off highway and limped to dealership. The technician replaced the master cylinder and noted the debris in the brake lines, therefore, he bled all the brake lines to clear the debris. There was no accident (luckily it was avoided), therefore, no insurance or police inspection. Only inspected by Ford dealership personnel. The only warning lamp that was illuminated was the tire pressure warning, since a spare tire was being used at the time.

Defect Description:
AIR BAGS
Potential Consequences:
My Passenger Seat Occupancy Sensor failed. Caused my airbag system to throw a code and become inoperative and SRS light on. Took to Middleton Ford in Middleton, WI and they confirmed the code indicated the OCS sensor has failed and required replacing to make the airbag system operable again. Meaning if I got into an accident, no airbags would deploy while the defective OCS sensor is in place. My insurance refused to inspect or cover the sensor as the failure wasn't related to an accident. Middleton Ford refused to cover the replacement cost regardless of cause of failure and said I must pay for the repair as my van was out of warranty. The sensor was replaced on 08/22/22 and see attached pics to prove the sensor had no visual issues and the cause of said failure is a manufacturer defect. Consumers should never have to pay for a defective part that failed due to no fault of their own.

WHEELS
Potential Consequences:
Husband took car to the local Ford dealer (Price Ford) to get oil changed. As part of the service, they went to rotate tires and found that the lug nuts/caps had swollen and needed to be replaced. We later discovered that this has been an ongoing problem for years. My husband agreed to have it done, for safety’s sake and to avoid having a flat and unable to change tire. This extra work cost $123.40. On later research, I found this design flaw was for cosmetic purposes only and that normal non-expanding lug nuts would avoid the distinct possibility that this may happen again. I called the shop and ascertained that, indeed, the “stock” lug nuts were replaced with same and he acknowledged that this has been a common issue. He also agreed that standard lug nuts wouldn’t do this and, finally, offered that if I went and bought them he would have them changed but also that he wouldn’t refund for the ones that were put on an hour ago because “now they are used “. My husband was never given the advice or option of using standard, less expensive, lug nuts in the first place, as he should have been. Why wouldn’t this be handled as a recall issue and standard parts be used for replacement, especially for customers who are experiencing this known problem? I feel a recall should be issued and remedied for all customers, free of charge. And, in our case, we should be refunded the $123.40.

Potential Consequences:
Driver-side cargo door does not latch correctly and will open while the vehicle is in motion. This happens at speeds below 20 mph as well as highway speeds. Because of the repeated pressure of the door flying open, the rail is now bent and the door will fall off when opened or closed. Not only is the cargo door opening a safety issue to others driving around me as things may fall out, but the door itself may come off and cause damage to those around me

Potential Consequences:
Need to replace swollen lug nuts on the vehicle. If I were to have a flat on the highway the tire would not be able to change it because the lug nuts are too big. No wrench will fit it. Service man said because the nuts are a 2 part lug nut, moisture gets in and make the nuts swollen and they get to big to take off with the wrench. Is the part defective? Advice me the cost from the dealer to replace. I read that the possible cause is tightening the lugs too tight. Not using the proper torque when changing tires. I have only Ford work on my car and they know it’s defective because it happened to my husband’s car and they covered it. They should be replaced by Ford. It’s DEFECTIVE. Thank you!

VISIBILITY:WINDSHIELD
Potential Consequences:
I kept hearing a banging sound which sounded like it was my ladder rack. So I took my ladder off until I could get it looked at. Which made no difference. And one day while driving I hit a bump in the road and the whole windshield lifted and slammed back down cracking all over. I didn’t know what to do so I was able to get some windshield adhesive and glue it back in however it’s still cracked in several places all over the windshield. And now I’m assuming it has to do with the recent recall. Are they responsible for the windshield? Thank you, [XXX] INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).

We have a 2016 Ford Transit Connect Van with 6,700 Miles. The Windshield come out loose and it could have been fatal to the life of passengers or others on the road. We went to the dealership and got to know that Ford is aware about it but is not covered. We are paying about $650 to glue the windshield back on the Van. We are paying this for the safety of the people. This could be dangerous and can lead to accidents or deaths on the road. Please address this with Ford since our concern is not being addressed by Ford or their dealership. Seems like they are waiting for a disaster to happen! Thank you.

Defect Description:
SERVICE BRAKES, HYDRAULIC:POWER ASSIST:ELECTRIC:CONTROL MODULE
Potential Consequences:
On October 15, 2021, I was driving my 2016 Ford Transit Connect at approximately 55MPH on a state road. I was closing in on a slow-moving truck, and pressed the brake pedal just to slow down about 5MPH. At this point, the brake electronic control module failed, locking up the left front caliper, nearly pulling me into oncoming traffic. I was able to get stopped and pulled over to the side of the road, where I found the affected brake (left front) was smoking hot. I shut the vehicle off and waited for it to cool, and upon restarted, the brake ECM allowed the caliper to open enough to drive it home, and then directly to the repair shop. The repair shop determined the brake ECM was randomly locking up one of the four calipers about 1 in 10 times. There were no warning indicators prior to the issue. The shop has all the details (Runyon Corp, PO Box 384/445, West Main St. Morristown IN, 46161, (765) 763-1271. They would have the old brake ECM. This issue occurred with 64,342 miles on the vehicle. I have contacted Ford, and was told other, similar vehicles of the same year and model had a recall on the brake ECMs, but my VIN was not included in the recall. This is a dangerous situation, and had I not had both hands on the wheel, it would have caused a head-on collision.
