FUEL/PROPULSION SYSTEM
Potential Consequences:
I was parked with engine on idling and the engine shut off. Tried to restart and it started shaking very rough idle, I turned off and tried to restart it would not start, I tried again and it started shaking again and shut off but the displays stayed on in the car. I called the Chevy dealer and had it towed to them. They ran diagnostics and confirmed it was an internal short inside fuel pump driver control module. They replaced and tested functional now. This is the second time they have replaced this part. The first time was last year driving when the engine decelerated and shut off. This is a safety issue and could be hazardous. My concern is this will happen again and I could be put in a very dangerous situation. I saw a bulletin issued for my vehicle with the same type of issue but my vehicle was not recalled to address Product Safety Recall N212332040 Stall While Driving Copyright 2021 General Motors. All Rights Reserved. Page 1 of 4 Release Date: October 2021 I am not sure if this is the same part or another issue.

FUEL/PROPULSION SYSTEM
Potential Consequences:
Service engine light came on, took it to Chevrolet dealership (2 months outside warranty). They found p2459 for regeneration frequency, performed necessary dpf regeneration procedure and was successful in completing and regeneration event, applied smoke to exhaust system for testing purposes and found large exhaust leak at rear tail pipe connection necessary to install new graphite seal and clamp. Also, found coolant leak at rear of engine on passenger side, upon further inspection found egr cooler coolant hose's actively leaking. necessary to replace hose's at this time. They do not want to release the vehicle back to us for “safety” reasons, while a part has been back ordered for 6 weeks. Doing some research, it appears there has been a trend of issues with the exhaust tail pipe, which is excluded also from extended warranty. I’m surprised, given the frequency of this on a newer vehicle that it has not yet been recalled. The coolant hose is a different issue, but likewise surprised it is not recalled.

Potential Consequences:
I have a 2021 Chevy Tahoe that has been to the dealer 3 times now for an airbag light (DTC B14DC). The dealer has yet to resolve this issue. I am being told it has to do with the harness in my drivers seat. The dealer performed a TSB for this issue which still hasn't fixed the problem. My wife and I travel back and forth from Michigan to Arizona and I am concerned that the system wont perform correctly if I am in an accident.

Defect Description:
ELECTRICAL SYSTEM
Potential Consequences:
2 ISSUES - 1) vehicle repaired 5 times for "transmission" problem by replacing wiring harnesses. On 5th attempt, the GM factory part or the technician caused the Tahoe to catch fire. Worst part is that it happened 1 hour after test driving and parking the car at their dealership, thankfully, I would normally have driven it home and it would have destroyed my home too. GM investigated and was unable to determine cause. 2) each time the "transmission" error occurred the GM car went into what they call the "limp" mode. The car immediately loses speed control and drifts to about 35 mph until you have control of the accelerator. I was on an interstate going +70 mph to pass a semi and immediately after passing the car went into Limp Mode, the semi nearly rear ended me and then passed me blowing his horn and cursing me. GM could certainly correct this latter issue by creating a warning on the screen that gives the driver ample time to pull over and address the problem as they do when DEF fluid (this was a diesel) is low and will give warning to add DEF or reduce speed after so many additional miles are driven. The former issue needs to be addressed immediately too, I've noted 4 other drivers on social media that had similar fires. Both of these issues could have resulted in serious injury to me or my family.

POWER TRAIN
Potential Consequences:
As a traffic crash reconstructionist, the issue is a MAJOR SAFETY ISSUE. My wife was exiting a residential driveway when the vehicle started to stutter and immediately shut down with no power or any capability to get the vehicle off the road. The shutdown caused the blockage of a residential street on a Friday afternoon, until the correct tow truck arrived. This was due to the fact the vehicle could not be moved due to not being able to start the engine nor the ability to place the vehicle into neutral, without starting the engine or crawling un der it with tools. It was determined the fuel module went bad. There was no warning of the failure of the component. Luckily the shut down of all components of the vehicle did not occur on a high speed or highly traveled road since I did not have any control of the vehicle after the fuel module failed. This is not the first report of the issue to NHTSA. Hopefully something will happen to find a fix to put the vehicle in neutral without tools, for the safety of the motoring public.
